道德商业行为分量表总共3道题，具体包括第1-3题；

利益相关者承诺分量表总共15道题，具体包括第4-18题；
其中一般承诺包括第4-6题；
社区承诺包括第7、8题；
消费者承诺包括第9-11题；
员工承诺包括第12-16题；
投资者承诺包括第17题；
供应商承诺包括第18题；

环境承诺分量表总共2道题，具体包括第19-20题；

可分别计算总分或平均分。
